Two different things with similar names

There is a real difference between a formal self-examination performed on a schedule as a screening method, and simply knowing your own body well enough to notice when something changes.

Organized guidance has moved away from the first. It has not moved away from the second. Nothing below is an argument for paying less attention to yourself.

Breast: what the trials found

Formal breast self-examination was taught for decades as a monthly routine. It was then tested in randomized trials, including one in Shanghai and one in Russia. The National Cancer Institute's summary of that evidence is direct: breast self-examination has been compared with no screening and has been shown to have no benefit in reducing breast cancer mortality.

The trials also found a cost. NCI states that instruction in formal breast self-examination leads to more breast biopsies and more diagnoses of benign breast lesions. The US Preventive Services Task Force went further, recommending against screening for testicular cancer in adolescent or adult men. It is a grade D statement covering both self-examination and clinician examination, issued in 2011. Its reasoning: the disease is uncommon, at roughly 5.4 cases per 100,000 males a year, more than 90 percent of newly diagnosed cases are cured, and most cases are already found incidentally. The Task Force concluded with moderate certainty that screening for testicular cancer has no net benefit.

None of that means a new lump, swelling, heaviness or change in a testicle should be left alone. It means the evidence does not support running a formal screening program in a situation where outcomes are already good and most tumors come to attention without one.

Skin: the organ you can actually see

Skin is the one place where you can look directly at the tissue in question, and the guidance reflects that.

The American Cancer Society describes how to do a skin self-exam: in a well-lit room, with a full-length mirror and a hand-held mirror, checking the whole body, including the scalp, back, buttocks, genital area, between fingers and toes, and the soles of the feet. It says doing one about once a month is usually suggested, with the right frequency depending on individual risk.

For moles and marks, the American Cancer Society sets out the ABCDE rule:

  • B, border. The edges are irregular, ragged, notched or blurred.

Other warning signs include a sore that does not heal, pigment spreading past the edge of a mole, redness or swelling beyond the border, new itching, tenderness or pain, and surface changes such as scaliness, oozing, bleeding or a lump.

On whole-body visual skin examination performed by a clinician as a screening test in people without symptoms, the Task Force issued an I statement in April 2023, meaning the evidence is insufficient to weigh benefits against harms. An I statement is not a recommendation against. It also does not apply to people who have symptoms or a suspicious lesion, who are evaluated regardless. NCI similarly notes that skin exams have not been shown to decrease the risk of dying from skin cancer.

The distinction, in practical terms

Across all three, the same line separates the two ideas.

A screening ritual is performed on a schedule, on the assumption that the searching itself finds disease earlier. For breast tissue that assumption was tested directly and did not hold.

Noticing and reporting change rests on a different basis: you are the only person who sees your body every day, and a change you can describe accurately gives a clinician something concrete to work with. That part has never been in question.

So if something is new, persistent, or different from your normal, describe it: what it is, where it is, how long it has been there, and whether it is changing. What happens after that is a clinical judgement, and it does not wait for a screening schedule.
