Side-Effect Sheet Review Checklist
For clinician review before a high-risk side-effect page is marked reviewed
Check urgency
- Does the page clearly say which symptoms should trigger same-day or urgent contact?
- Is fever, bleeding, breathing trouble, chest pain, severe diarrhea, rash, or confusion handled safely when relevant?
- Is the language strong enough without causing unnecessary panic?
Check claims
- Every medical claim matches the cited source or is removed.
- No dosing, self-treatment, supplement, or delay-care advice is implied.
- Patient questions are useful and realistic for clinic workflow.
Only after review
- Add reviewer name, credentials, role, review date, exact reviewed path, and conflict note.
- Do not use a medical-review label for general site feedback or unpublished drafts.
