Insurance Appeal Letter Template
A fill-in starter for denied cancer care, medicines, scans, or supportive services
Include
- Patient name, member ID, claim or authorization number, denial date, and service denied.
- The exact reason listed in the denial letter.
- The requested service, prescribing clinician, diagnosis, and deadline.
Ask the care team for
- A letter of medical necessity if they think the service is appropriate.
- Relevant visit notes, test results, guidelines, or prior treatment history.
- Whether the appeal should be standard, expedited, internal, or external review.
Track
- Date sent, method sent, fax confirmation or upload receipt, and person spoken with.
- Every reference number and next deadline.
