Fact-Checking Joe Rogan and David Sinclair's Latest Anti-Aging Claims
A viral JRE episode covers an FDA-cleared eye gene therapy, Viagra and cancer, and NMN. Here's what NIA, FDA and clinical trial records actually show.

David Sinclair is a Harvard genetics professor and longevity researcher. He returned to The Joe Rogan Experience in early August for his fourth appearance (Episode #2537). Clips from the two-hour-plus conversation have circulated widely since. The episode ranges from an experimental eye gene therapy to erectile-dysfunction drugs, NAD+ supplements, and multi-day fasting. Some of it holds up well against the public record. Some of it needs real context.
Claim: "this trial is FDA approved" — a gene therapy that reboots aging cells in the eye
Sinclair describes a treatment he calls ER100 ("Epigenetic Reprogramming 100"). It is delivered via an engineered virus injected into the eye. It uses three genes — OCT4, SOX2 and KLF4, nicknamed "OSK" — to partially reprogram aging cells back toward a youthful state. He says "this trial is FDA approved" and that it's being tested first in glaucoma and a related optic-nerve disease.
This checks out. It's a genuinely notable development. Sinclair's company, Life Biosciences, received FDA clearance in January 2026. That let it begin the first human trial of this therapy. It dosed its first patient in mid-2026. The trial (ClinicalTrials.gov ID NCT07290244) is a small Phase 1 safety study — the earliest and most cautious stage of human testing. It enrolls around a dozen adults with open-angle glaucoma or a related optic neuropathy. Its primary purpose is to check whether a single injected dose is safe and tolerated. It is not meant to prove the dose restores vision.
The science Sinclair describes is also accurate. The four "Yamanaka factors" were discovered by Shinya Yamanaka. He won the 2012 Nobel Prize for showing they can turn adult cells all the way back into stem cells. Fully reprogramming cells that way is known to risk tumors called teratomas. That is a real, well-documented safety concern. That is why Sinclair's approach leaves out one factor, c-MYC. It stops short of full reprogramming.
The clip runs ahead of the evidence in one place: the leap from "safe in a small eye trial" to "whole body rejuvenation." Sinclair explicitly describes that leap as the eventual goal. A 12-person, single-dose, eye-only safety trial that hasn't reported results is a long way from any of that. It isn't designed to test it, either.
Claim: erectile-dysfunction drugs may fight cancer
Discussing tadalafil (Cialis) and sildenafil (Viagra), Sinclair says "a paper just came out about its role in preventing cancer." He says the drugs make certain tumors more visible to the immune system. He also says sildenafil blocks cancer cells' ability to use cholesterol.
This is a real, active research area. It is not something invented for the podcast. Published studies, some a decade old and some from 2026, have found tadalafil can reduce immune-suppressing cells around head-and-neck tumors. A study published in Cancer Research in August 2026 found sildenafil interfered with cancer cells' cholesterol handling. That study also found lower five-year mortality in men who'd filled multiple sildenafil prescriptions before a cancer diagnosis.
That said, the human data so far is observational. It is an association seen in men already taking the drug for ED. It is not a controlled trial proving the drugs prevent or treat cancer. Neither drug is FDA-approved for cancer prevention or treatment. The studies' own authors describe the findings as a basis for further research, not a treatment recommendation.
Claim: NMN and resveratrol as everyday anti-aging molecules
Sinclair says he takes NMN "to mimic" the stress response of exercise. He describes his elderly father's daily regimen of NMN, resveratrol and metformin as a model of healthy aging.
The National Institute on Aging runs the most rigorous test for this kind of claim. It is the Interventions Testing Program (ITP). The ITP feeds compounds to genetically diverse mice at three independent labs. It measures whether they actually live longer. According to NIA's own published results, resveratrol was tested twice, at two different doses. It produced no increase in median or maximum lifespan either time. Nicotinamide riboside is a close chemical cousin of NMN. Like NMN, it is a precursor the body uses to make NAD+. It was also tested. It likewise showed no lifespan extension. NMN itself has not yet appeared in the ITP's published results.
Separately, NMN's legal status as a supplement has been unusually turbulent. The FDA excluded it from the legal definition of a dietary supplement in 2022. That was because a drug company had already begun developing it as a prescription drug. The FDA then reversed that position in 2025. NMN became legal to sell again. That back-and-forth reflects an unsettled regulatory question. It is not evidence either way about whether NMN works in humans.
Claim: multi-day fasting and "deep cleansing" through autophagy
Sinclair recommends fasting for three days, or even a week. He describes a process called "chaperone-mediated autophagy" that "cleans out a lot of the old proteins in your body." He says he does it himself "as an anti-aging thing."
Autophagy is the cell's process for breaking down and recycling its own damaged components. It is real, well-documented biology. Fasting is one way to increase it. But no human clinical trial shows that a three-day or week-long fast extends human lifespan or reverses aging. Multi-day water-only fasting also carries real risks around blood sugar, electrolytes and muscle loss. Those risks rise with the length of the fast. Bottom line
Two claims in this clip are accurately described: the FDA-cleared eye gene therapy and the emerging ED-drug/cancer research. Both reflect real, ongoing, government-registered science. Even so, both are early-stage and unproven at the level of an approved treatment. The supplement claims run considerably ahead of NIA's own controlled lifespan data. That data hasn't found a benefit for resveratrol, or for NMN's close chemical relative, in rigorous animal testing. And the multi-day fasting recommendation is grounded in real biology. But it is presented without the caveats that would normally accompany a personal health experiment of that length.
