What is a follow-up care plan?

A follow-up care plan is a summary of your cancer treatment along with the next steps for your care. Asking for one can give you a sense of control after treatment ends.

Having this summary can make follow-up visits and tests feel more manageable, especially when coping with worry about recurrence. It also helps you keep track of which symptoms to report and when your appointments are.

Ask your care team whether they can provide a follow-up care plan, and bring your questions to your visits.
